Insurance Information

Our education program meets Medicare guidelines and is covered by most insurance carriers. Please check with your insurance company for details of your coverage.  Co-pays, if applicable, will be collected at the time of the appointment.

Scheduling an Appointment

Appointments for diabetes self-management education (individual and class) and for medical nutrition therapy require a provider order sheet signed by your primary care provider.

We don’t require the order sheet for appointments for medical management with the nurse practitioners.  However, your insurance may require pre-authorization from your physician.  You may also need an insurance referral.

Medicare does not cover diabetes education or medical nutrition therapy visits for prediabetes.
